==Accidents== The Cleveland Dam had several failures of the drum gate, causing water to be uncontrollably released into the Capilano River running through Capilano River Regional Park. Warning sirens were not installed until after the death of two men in 2020.<ref name=":0">{{Cite web |last=Vancouver |first=Metro |title=Cleveland Dam Safety Enhancements Program {{!}} Metro Vancouver |url=https://metrovancouver.org/services/water/cleveland-dam-safety-enhancements-program |access-date=2024-02-13 |website=metrovancouver.org |language=en-US}}</ref> In 1975, a nine-year-old girl was killed by a surge of water released from the dam.<ref name="ctv.worksafe">{{cite web |last=Woodward |first=Jon |date=2020-10-06 |title=Order to improve safety at Cleveland Dam apparently ignored, despite several other failures |url=https://bc.ctvnews.ca/order-to-improve-safety-at-cleveland-dam-apparently-ignored-despite-several-other-failures-1.5135595 |access-date=2020-10-08 |publisher=CTV News}}</ref> The area had no warning signs or alarm at the time. Signs were later added to warn visitors of the potential hazards of the dam.<ref name="ctv.worksafe" /> In 2001, four fishermen were trapped in the river when the water level surged without a warning after the spillway's drum gate inadvertently dropped open.<ref name="ctv.worksafe" /> [[WorkSafeBC]] issued several orders to reduce the risk to workers and the public, including but not limited to: "spillway gate lockout, access to the riverway by workers where lockout is not required, mancheck systems, emergency rescue and evacuation, public warning signage, and warning alarms".<ref name="ctv.worksafe" /> However, some of the orders were ignored and public-facing warning alarms were never installed.<ref name="ctv.worksafe" /> On October 1, 2020, the dam's drum gate opened unexpectedly during maintenance, causing water to gush out onto Capilano River. The torrent of water killed one person with another person missing and presumed dead.<ref name="GS 2020-10-30">{{Cite news|last=Dunphy|first=Martin|date=2020-10-30|title=Three workers fired in aftermath of Cleveland Dam incident that resulted in one dead, one missing|url=https://www.straight.com/news/three-workers-fired-in-aftermath-of-cleveland-dam-incident-that-resulted-in-one-dead-one|access-date=2021-02-03|work=The Georgia Straight|language=en}}</ref> Several park goers were trapped within Capilano River Regional Park.<ref name="cbc.torrent">{{cite web |url=https://www.cbc.ca/news/canada/british-columbia/cleveland-dam-witness-report-1.5747888 |title=Witness to fatal torrent released from B.C. dam recounts moment 'all hell broke loose' |date=2020-10-01 |publisher=CBC News |access-date=2020-10-02 }}</ref> While many managed to escape by themselves, four people had to be rescued.<ref name="global.missing">{{cite web |url=https://globalnews.ca/news/7372931/water-rescue-underway-near-north-vancouvers-capilano-suspension-bridge/ |title=Crews search for missing person after dam opened during maintenance, flooding Capilano River |last=Little |first=Simon |date=2020-10-01 |publisher=Global News |access-date=2020-10-02 }}</ref> The Capilano River's water level rapidly increased by {{convert|3.58|m|ft}}, pushing {{convert|413|m3|cuft}} of water per second at its recorded peak.<ref name="cbc.torrent" /> These flow rates normally only occur in the rainy season. The anglers at the river edge had no warning except the sound of rushing water as the dam has no public alarm system to warn of a water surge.
