In April 2019, repairs were finished and it was determined that the cause of the cracking was partly caused when crews welding the beams together skipped a crucial step in the process required by the building code that led to tiny, micro-cracks forming. Multiple inspections failed to notice the skipped step, and those micro-cracks grew into larger ones.
